Guide to Free-form Scleral Lenses

Free-form Scleral Lenses

Free-form scleral lenses are a completely customized scleral lens. It is designed from sMap3D corneo-scleral topography measurements of your eye rather than from a standard set of diagnostic lenses.

Because the design exactly matches the actual shape of the cornea and sclera, the lens is customized to fit each individual eye. This is useful for patients who need a precise fit for comfort, and for eyes that are difficult to fit with more conventional scleral lens designs.

How free-form scleral lenses are different

Most scleral lenses use a pre-designed system of peripheral curves. Free-form scleral lenses do not. The lens is designed from the elevation data collected by the sMap3D topographer, so the landing zone and clearances are shaped to the individual eye.

Dr. DeNaeyer uses this approach for patients who may benefit from a fully contoured fit, including those with:

  • Significant corneal irregularity (keratoconus, post-transplant, trauma)
  • Marked scleral toricity or asymmetry
  • Eyes that have not been comfortable in other scleral designs
  • The need for a precise match between the lens and the ocular surface

What the visit involves

The first visit includes a medical eye exam, computerized corneal maps, sMap3D corneo-scleral topography, and photography. The sMap3D measurements are required to design free-form lenses; the lens cannot be ordered from diagnostic lenses alone.

Production typically takes 10–14 days, followed by a dispense appointment to confirm the fit and teach insertion and removal.

Who is a candidate

Free-form lenses are an option when standard scleral lens designs are not expected to be successful. It is not required for every patient. Many people do well with other custom scleral designs fitted with the same mapping technology. The choice depends on the shape of the eye and the goals of the fit.
